ACO Transformation Specialist

Job Summary

The ACO Transformation Specialist supports the vision of the Total Healing Environment and the values of respect, integrity, accountability, commitment and quality by creating and supporting population health programs for the Accountable Care Organization (ACO). Acts as a resource and content expert for education to support clinical process improvement and data analysis to track quality goal attainment.

Job Qualifications

Required Qualifications:

  • Associate's degree in Business, Healthcare, Education, Information Systems or related field
  • Experience with data analysis and quality metrics
  • Experience creating and conducting training and education for clinical and non-clinical staff
  • Excellent presentation/facilitation skills including the ability to facilitate focus groups and subject matter expert interviews and identify/document key themes emerging from these activities
  • Ability to work independently, set goals and complete tasks and projects on time
  • Basic project management skills
  • Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service
  • Excellent organizational skills and the ability to handle multiple priorities/tasks simultaneously
  • Computer literacy and proficiency in Microsoft Windows
  • Proficiency with MS Office (Outlook, Word, Excel, PowerPoint)

Preferred Qualifications:

  • Bachelor's degree
  • Provider relations, medical office or hospital experience
  • Familiarity with Accountable Care Organizations
  • Understanding and demonstrated ability in use of process improvement techniques (ie: PDSA, LEAN six sigma)
